Home Health Care
Mercy at Home
Before modern medicine, caring for someone medically
in the home was quite common, and hospitalization was rare. Today, with
a growing trend toward reduced hospital stays, home health care is
gaining popularity.
While caregivers of the past were generally family
members or friends, home health care today is based more on a
professional health care team working toward a common goal.
The home health team at Mercy (known as Mercy at
Home) consists of nurses, nurse aids, physical therapists, social
workers and clerical support. Home health care may be considered for
patients who are recovering from an illness, undergoing treatment,
disabled, chronically ill or terminally ill.
Mercy at Home offers services including:
-
Adult care nursing
-
Physical, occupational and speech therapies
-
Mother / baby and pediatric care
-
Tel-health (long distance monitoring used for
congestive heart failure and high risk maternal care)
-
Management of chronic diseases (COPD, CHF,
diabetes)
-
Registered dieticians for consultation and
nutritional support
-
Fall risk prevention
-
Wound care
