Medical Records
Copies
of your medical records for services performed at Mercy Memorial may be
obtained by contacting our Health Information Management Department.
Requests can be made in person or in writing. Please be sure to
include your full name, other possible names used, birth date, social
security number, phone number, type of services received and date of
service.

Copy
charges are 25 cents per page. Please allow 10 working days for requests
to be filled.
Or
you may sign a release of information in the Health Information Management
office, and allow three days for requests to be filled. If you have
questions about the release of medical information, contact the Health
Information Management office.
Due
to federal privacy laws regarding the release of medical information, we
are not able to answer requests for medical records made through the
Internet.
